Service Request Form
If sending units back include a copy of the form with the returning package. The Cryopak Service Department will evaluate the received units and provide a service quote for your approval. If expedited service is required, please discuss with your sales rep or service technician. If you need to return defective goods to Cryopak Digital, Inc. USA, Please complete all appropriate sections of this form and return it together with the product.
- Kindly complete one form for each returned product.
- Please do not complete separate form for similar type of units with same fault or requirement.
- Please do not separate peripherals like external sensors, if they are being sent with a logger.
